总结了一些最近认证的东西，爬了很多文，以下是现在该怎么申请的方法。最下面有一
些资料，不愿意看的直接看结论。我是完全刚刚开始认证。所以这是给刚报名的大家看的。



第1步~从学校寄出去ECFMG

建议自己寄，但是地址要写学校。

(1)表186一份(要贴照片，要有official签字)
(2)两个�印学位证
(3)官方学位翻译一份(何�官方，就只好自己去找翻�社ECFMG承�的)
(4)照片一张(不算上186的那一张)
(5)表344一份
(6)表345两份
(7)两份成绩单�印件
(8)表187一份


***请注意***

可能你会被表345�面的第2,3页提到的搞混，不过!!!!这是FCFMG寄回给你的学校之后
，你才要处理的。

• An official, final medical school transcript which bears your
institution’s seal and the signature of an authorized
official; and
• Certification of the enclosed Final Medical Diploma, by affixing the
institution’s seal and the signature of an
authorized official onto the diploma; and
• An authorized official of your Medical School to complete the
attached
form titled Verification of Medical Education.

这是说需要你的

(1)成绩单有盖章签字(都要186的那个Official签字)
(2)学位证明有盖章签字(都要186的那个Official签字)
(3)应该是ECFMG寄的327表

第二步~ECFMG会寄327表345表等等资料到你的学校

应该是包括�面大家说的327表345表，还有你的成绩单等等之类的东西。
需要你的

(1)成绩单有盖章签字(都要186的那个Official签字)
(2)学位证明有盖章签字(都要186的那个Official签字)
(3)应该是ECFMG寄的327表

怕ECFMG寄到�校速度不�?�看最下面�料中Third的部分

不��於学校对ECFMG寄过来的东西，会不会处理?要怎么加快速度?
怕学校教务处忽略那些信?或者是学校会不会帮你弄那些东西?

这个我目前还不知道怎么解决，可能要有同学在学校那边监督吧~
然后请同学帮你跑，如果学校不愿意帮你办的话。

第三步~再从学校寄到ECFMG

把�面ECFMG这次寄给学校教务处的交代的事情办完之后，就可以寄回给ECFMG了。
如果�校他们不帮你寄，一样，还是老方法，寄件地址改学校自己寄到ECFMG。

这样应该就可以完成认证了，如有错误欢迎指证。

以下是资料，是我在报名的时候跳出来的，我想BOOKLET还有表的内容大家都能自己找
到，
所以我附上的是报名的时候秀出来的东西，这些就不是随便找找到的了。

First,首先这是报名时候的选项

I have graduated from medical school and am enclosing, with my
Certification of Identification Form (Form 186), the IWA Document
Submission Form (Form 187); the ECFMG Medical Education Credentials
Submission Form (Form 344); two copies of the Medical School Release
Request (Form 345); two photocopies of my medical diploma with an
English translation (if required); and a photograph. If my Form 186 is
signed by an authorized official of my medical school, the envelope
containing the above-listed documents will be sent to ECFMG directly
from the office of that official.

• The following must be sent to ECFMG directly from the office of
the medical school official who certified the Certification of
Identification Form (Form 186):
1. Original, signed Certification of Identification Form (Form
186).
2. Two photocopies of medical diploma (if not previously submitted)
and official English translation of medical diploma (if diploma is not
in English); one current, original, full-face, passport-sized, color
photograph (this photograph is in addition to the photograph that
applicants must send with the Certification of Identification Form); the
ECFMG Medical Education Credentials Submission Form (Form 344); and two
copies of theMedical School Release Request (Form 345).
3. If applicable, two copies of an official transcript issued by
any school or institution from which you transferred credits to the
medical school that awarded your medical degree (if not previously
submitted); official English translation of transcript (if transcript is
not in English); and the ECFMG Medical Education Credentials Submission
Form (Form 344). If you are also submitting copies of your medical
diploma, you only need to submit one Form 344. See Transfer Credits and
Credentials for ECFMG Certification in the ECFMGInformation Booklet.
4. Any additional documents that are required to complete your
application.
5. A completed IWA Document Submission Form (Form 187) if, in
addition to your Certification of Identification Form (Form 186), there
are any additional documents required to complete your application.




Third,

楼下有同学谈到ECFMG认证材料无法投递到学校的情况，煞是头疼。我遇到过同样情况
，可以理解那
种心急如焚的心情。我是这样处理的：

1。申请FEDEX账号，有信用卡即可。

2。打电话给ECFMG，说你希望他们用你自己的FEDEX账号邮寄你的材料。他们会给你一
个传真号
码。你起草一份正式的授权信，打印，签名，然后传真给他们，授权ECFMG用该账号特
快邮寄你的材
料(所有费用由你出)。与你的ECFMG证书的重要性相比，这几十块钱还是毛毛雨了。用
速递的其他好
处就是快(时间就是金钱)，而且可以追踪。

3。1周左右打电话给ECFMG询问TRACKING NUMBER，得到TRACKING NUMBER以后查询邮包状
态。
通常ECFMG系统内中国医学院的地址是不准确的，如果有合校情况就更加如此。但你无
法说服ECFMG
把邮件寄到你提供的地址(因为如果他们允许这样作，就会给作假者有可乘之机)；而
FEDEX按ECFMG
提供的地址基本上是无法投递的。他们会继续试2-3次，然后按“无法投递”退回美国。

4。打电话给你学校问清楚正确地址和相关接收老师的姓名，电话。

5。打电话给国内FEDEX的分公司(学校所在地的联邦特快)，告诉他们你的邮包的
TRACKING
NUMBER，无法投递的原因，和正确的地址，老师姓名和电话。现在国内速递公司的态度
都很好，懂
英语的年轻人很多，只要你态度好点耐心解释，他们会帮助你的。(速递公司肯帮你，
估计是因为如果
邮包无法投递送回美国的话他们损失更大，呵呵)

6。然后就继续查看邮包状态吧，一般下一次投递就会成功的。祝各位好运！
